Live Well with Chronic Conditions is a 6-week workshop for individuals with a chronic condition and/or their caregivers. Participants will learn how to manage their ongoing health issue using Action Planning, Decision Making and Problem Solving. Topics covered in the 2.5 hour sessions include managing fatigue, healthy eating, physical activity, managing stress and difficult emotions, and communicating effectively. Originally developed at Stanford University, this evidence-based program is beneficial in helping individuals take control of their chronic condition.
Workshops will take place on Wednesdays from 10:00 – 12:30pm beginning April 10th.
